General Instructions

1. Change Initiators - Complete all fields except the completion log. An empty priority field is accepted.
2. Enter a good summary of the change description. Provide more details in an email.
3. Priority Levels are : Low, Medium, High and Critical. Choose them wisely as we have limited resources and everybody choosing a higher level than required will not help.
